DMA 4th of July Games Sign-up

This is a Douglas Manor Association sponsored event. To participate you must be a DMA or Douglaston Club member or be sponsored by a member of either organization. Please complete the form below to confirm your family will participate in the games. It's gonna be a fun day for all!

Swim Events will include Free, Back, Breast Fly & IM
Field Events will include Basketball, Softball, Long Jump, Short Run and Long Run.

Cost: $25.00 per child or $45.00 per family. Please make the check payable to Douglas Manor Association and put DMA 4th of July games in the memo. Send the check to the office at 39-50 Douglaston Parkway, 2nd fl. Douglaston, NY 11363.

Release Disclaimer
I assume the risk of all injuries, damages, or loss that I or my invitees may sustain as a result of attendance at or participation in the DMA 4th of July Games. I agree to release, hold harmless, and not to sue or make a claim against the Douglas Manor Association and its directors, officers, employees, members, and agents for all liabilities, losses, claims, actions, demands, damages, costs, and expenses arising from or in connection with my or my invitees’ attendance at or participation in the DMA 4th of July Games, including, without limitation, any loss of, damage, or injury to any property or to any person, whether caused by the acts, omissions, or negligence of the Douglas Manor Association or its directors, officers, employees, members, and agents, or otherwise.

Photo Permission Disclaimer
I grant permission to the Douglas Manor Association to use any images or videos of myself or my invitees captured during the DMA 4th of July Games, to be used solely for the purpose of Douglas Manor Association promotional materials, publications, and photo galleries, and waive any rights of compensation or ownership thereto. I further agree that these images of myself or my invitees may be included in brochures, posters, websites, social media, and print media for the Douglas Manor Association.

Event Volunteer
We are also in need of volunteers to help run the games. If you have aged out of the games but still a kid at heart and want to help with the DMA Games please register here. We could use approximately 10 volunteers for Swim Events and 20 for Land Events. Join the fun for one or both days!
